We are very excited to announce that MyChart is coming to Trillium Health on May 1, 2025! MyChart is the same patient portal system used by UR Medicine.
The switch to MyChart will make it easier for patients and providers to communicate with each other. Once enrolled, you will be able to contact your medical team, access your test results, request prescription refills, and manage your appointments in MyChart beginning on May 1, 2025.
Trillium Health’s old patient portal will be closing at midnight on April 28, 2025. If you need to contact your providers, request an appointment, or refill your prescriptions between April 29-30, please call Trillium Health at 585.545.7200.

How will I access MyChart?
Beginning on May 1, 2025, you can access MyChart the same way that you accessed the old portal: go to trilliumhealth.org and click on the “patient portal” button at the top of the screen. You can also visit trilliumhealth.org/mychart.
I already use MyChart. Will I need to set up another account?.
No. If you already use MyChart, you can use the same account.
When will I start to see Trillium Health appointments in MyChart?
If you currently use MyChart with UR Medicine, you may start to see Trillium Health appointments in the next few weeks. This is normal and it’s an example of how MyChart will make it easier to track your care between health organizations.
I don’t currently use MyChart, but I’d like to start. How can I get an account?
Visit trilliumhealth.org/mychart. Then click the blue button that says, “Sign Up (I Don’t Have a Code).”
Can I use the same login as my old patient portal?
No. You will need to set up a new account in MyChart.
