March 8, 2024 (Rochester, NY) – Trillium Health is proud to announce that Pathway Pediatrics, a division of Trillium Health, has been awarded a Patient-Centered Medical Home (PCMH) Recognition from the National Committee for Quality Assurance. Additionally, Trillium Health’s main clinic, located at 259 Monroe Avenue, has renewed its designation as a PCMH for primary care.

A patient-centered medical home (PCMH) is not a place – it is an approach to delivering high-quality, cost-effective primary care. The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality measures, and preventive care. The National Committee for Quality Assurance established the PCMH Recognition program in 2008.
