Rochester, NY – Trillium Health, a community health center with an emphasis on affordable health care and located in downtown Rochester, welcomes board certified Nurse Practitioners Diego Grisales and Elissa Hughes to the medical practice.
Diego Grisales, NP joins Trillium as a Family Nurse Practitioner with a specialty in Family medicine, LGBT health care and HIV prevention. Grisales has experience working in geriatrics and improving health care for migrant workers in the Finger Lakes.
Elissa Hughes, NP joins the care coordination team as a board certified Family Nurse Practitioner. Hughes diverse experience includes Internal Medicine, Endocrinology, Psychiatry and Hospice.

Grisales joins Trillium from ElderONE, a geriatric clinic affiliated with Rochester General Health System. He has master’s degree as a nurse practitioner from St. John Fisher College and a bachelor’s degree in nursing from Nazareth College. Originally from Medellin, Colombia, he lives in Rush, NY.
“As a provider at Trillium Health I hope to be a tool for insight for improved health for our patients” says Hughes.
Hughes is a native Rochesterian with diverse experience as a Registered Nurse and continued her education, becoming a Board Certified Nurse Practioner. In addition to her Masters of Science from University of Rochester she holds a Bachelor’s in English Literature and Technical Writing Nazareth College.
