November 27, 2023 (Rochester, NY) – Trillium Health is proud to announce that it has won two quality awards from the Health Resources & Services Administration (HRSA), a subagency of the U.S. Department of Health and Human Services. These awards recognize Trillium Health's outstanding achievements in addressing social risk factors and advancing health information technology.
Addressing Social Risk Factors:
Trillium Health screens patients for their social risk factors using a Social Determinants of Health (SDOH) screening tool. The percentage of patients who received care management and supportive services increased between 2021 and 2022.
Advancing Health Information Technology (HIT)
Trillium Health uses a system for Electronic Health Records (EHR), offers telehealth services, exchanges health information electronically, engages patients through a patient portal, and collects data on social risk factors.

The awards were distributed through HRSA’s Community Health Quality Recognition Overview, which is based on data from the latest Uniform Data System (UDS) reporting period.
