At Trillium Health, we have a comprehensive model of care – we treat the physical, emotional, and social determinants of health of our patients. Barriers to housing, transportation and food make it extremely hard to focus on your health, so our supportive services are here to help with that!

food pantry

Whether you stop by every month or for a one-time visit, our food pantry can help get you through.

About the Food Pantry:

  • It's open to anyone who has a need for food

  • You can visit the Food Pantry once a month

  • We offer fresh fruits & vegetables, canned goods, frozen foods, and more

To schedule an appointment at our Food Pantry, please call 585.545.7220. We can also accept walk-ins during open hours.

  • Monday: 1:00 pm - 4:00 pm
  • Wednesday: 9:00 am - 12:00 pm & 1:00 pm - 3:00 pm
  • Thursday: 1:00 pm - 4:00 pm
  • Friday: 9:00 am - 12:00 pm & 1:00 - 3:00 pm

Trillium Health’s Food Pantry is in partnership with Foodlink, a community food resource center. Together, we work to create a more nourished, prosperous community.

connections to emergency housing

Dealing with housing instability can be very hard to handle alone. In our Housing Services program, we do everything we can to make sure you get the help you need. Our goal is to keep you from becoming homeless and to connect you to health care.

People who are living with HIV or are experiencing homelessness may be eligible for housing services available through the programs below.

  • Housing Opportunities for Persons With AIDS (HOPWA) helps people who are HIV-positive with their housing needs. HOPWA is available for people who live in Monroe, Wayne, Ontario, Livingston, Orleans, Seneca, Yates, Schuyler, Steuben, and Genesee counties.

    We can help with:
    • Getting started: first month’s rent, security and utility deposits, and application costs
    • Rent subsidies: HOPWA will pay the difference between 30% of your gross monthly income and fair market rent
    • Emergency help: past-due rent, utility shut-offs, and mortgage help

  • Trillium Health, in conjunction with Empire State Supportive Housing Initiative (ESSHI), and Home Leasing, are partnering on a housing project called Union Square located at 275 East Avenue (on the former Inner Loop).

    To be eligible, individuals must:

    Currently be homeless AND
    - Be living with HIV OR
    - Be age 55+ and require assistance with one or more Instrumental Activities of Daily Living (IADL)*

    *What are the IADLs?

    - Independent living skills (e.g. house cleaning, home maintenance, shopping, and meal preparation)
    - Managing finances and benefits (e.g. budgeting, paying bills, benefit and insurance enrollment)

transportation services

Trillium Health’s transportation program ensures that transportation is not a barrier when it comes to your needs. And you don’t have to be a Trillium Health patient to participate in the program! The transportation program offers support such as bus passes, gas cards, and subcontracted transport (taxi, van).

Trillium Health's transportation program may be right for you, if you:

  • Are living with HIV and unable to obtain Medicaid funded transportation

  • Need to obtain supportive services that are intended to improve medical outcomes

Transportation is not provided for running personal errands or travel to non-scheduled appointments. 

Join the program
If you want to join Trillium Health’s transportation program, please have your doctor or care manager tell us that you want to join (a referral). If you are not a patient, call us to ask about joining. Once you are enrolled in the program, you must maintain eligibility by completing a reassessment every 6 months (proof of income, residency and medical insurance)

nutrition health education program

We can help you take charge of your own health as you learn how to eat healthy on a budget. This program will teach you how to plan, shop, save, and prepare healthy foods that are just right for you.

Trillium Health’s Nutrition Health Education program is for you, if you:

  • Have HIV

  • Need help with eating healthy

  • Have another condition, like diabetes, high cholesterol, or high blood pressure

If you want to join Trillium Health’s Nutrition Education Program, please have your doctor, care manager, or dietitian tell us you want to join (a referral). Then we will meet with you to find out what you need and get you started. Call us for more information.

care management services

Care Management, or care coordination, is a service that helps people achieve their goals by helping them find the services they need. If you would like help accessing things like our Food Pantry, housing support, and transportation services, you might be a good fit for our Care Management program!
