our Hepatitis C Center of Excellence
Up to 75% of people living with Hep C don't even know they have it. Those born between 1954-1965 are at an especially high risk for infection.
ARCH (Age-Related Changes with HIV)
ARCH is a program that provides services to people living with HIV who want to thrive in the second half of their lives.
If you are a person living with HIV, are age 50+ and starting this next chapter of your life, our team is here to listen and create a plan that focuses on what matters most to you.
Our ARCH team offers:
- Our Ryan’s Connection Center, a place to go that provides community and belonging. You can meet new people, learn, or just stop in to have a cup of coffee and engage in conversation.
- Health education focused on topics important to you as you enter your golden years
- Opportunities to learn about Medicare and have help selecting the plan that best meets your needs
- Wellness screenings to identify changes in your mobility, memory, and emotional health
- Personalized support with managing multiple medical concerns
- Assistance guiding you through the medical system
- Support for you and your family with planning for your future health care and making decisions based on your wishes
- Medication reviews to ensure your treatment supports your best health
- Referrals for supportive services (food, housing, and transportation)
