Trillium Health’s transportation program ensures that transportation is not a barrier when it comes to healthcare. The transportation program offers support such as bus passes, gas cards, and subcontracted transport (taxi, van).
Trillium Health's transportation program may be right for you, if you:
Are living with HIV and unable to obtain Medicaid-funded transportation
Need to obtain supportive services that are intended to improve medical outcomes
Transportation is not provided for running personal errands or travel to non-scheduled appointments.
If you want to join Trillium Health’s transportation program, please have your doctor or care manager tell us that you want to join (a referral). If you are not a patient, call us to ask about joining. Once you are enrolled in the program, you must maintain eligibility by completing a reassessment every 6 months (proof of income, residency and medical insurance).
