ROCHESTER, NY. (July 25, 2016) – Trillium Health meets the eligibility requirements of the Health Resources and Services Administration (HRSA) Bureau of Primary Health Care to provide expanded primary care to patients regardless of their ability to pay.
Trillium Health is located at 259 Monroe Avenue at Monroe Square in downtown Rochester, and provides complete primary and preventive care to neighboring communities, including the uninsured, patients with commercial insurance, Medicaid or Medicare. Services include primary & mental health care, chronic disease management, care management, specialized Hep C, HIV and STD care & treatment, referrals to specialists, laboratory services, insurance enrollment, supportive services, food bank distribution and more. Our On-site pharmacy – open to the public, and evening hours, same-day and walk-in appointments are available in addition to advanced scheduling.
Federally Qualified Health Centers provide high quality preventive and primary health care to patients regardless of their ability to pay. An FQHC-Look-Alike is an organization that meets all of the eligibility requirements of an organization that receives a Public Health Service Section 330 grant, but does not receive grant funding. With Look-Alike designation, Trillium Health will receive enhanced Medicare and Medicaid reimbursement, reduced pricing on prescription and non-prescription medications for our patients, and improved access to qualified health care providers.
